PeaceHealth partners with Opportunity Village

Residents at Opportunity Village are now receiving on site medical care!

Beginning in July, PeaceHealth Medical Group's Unified Care Clinic (UCARE) has been making bi-weekly visits to the Village. The clinical team—which includes a nurse practitioner, case manager, patient access representative, mental health therapist, and peer support specialist—has helped villagers enroll in the Oregon Health Plan, access vision and dental care, manage diabetes, and receive preventive medical care like immunizations.

"We're a clinic that provides easily accessible and holistic care. We try to take care of everybody and meet them where their at," says UCARE's peer support specialist Jonah.

The clinic provides lunch when it comes to Opportunity Village, helping them build relationships with people who have been underserved by the medical system. "We have been able to work with people who, based on their experiences, don't trust the medical system and therefore don't seek medical care," explains nurse practitioner Kathryn Kernan. "We break that barrier by coming to people where they're comfortable," she adds, "We've been able to bring people care who otherwise would have ended up in the Emergency Room."
